OFFICIAL RULES FOR Quantum health's "Healthy mouth, Happy mouth" Dental care Woman's world MAGAZINE Sweepstakes
NO PURCHASE IS NECESSARY TO ENTER THE SWEEPSTAKES
SPONSOR: The Healthy Mouth, Happy Mouth Sweepstakes (the "Sweepstakes") is sponsored by Quantum Health ("Sponsor").
PRIZE: $1,000 (One thousand dollars) worth of dental care and $50 worth of Quantum Health's oral care products, including OraMoist, Canker Cover and Lip Clear Lysine+; approximate retail value of total prize is: $1,050 (ARV). The $1,000 prize will be placed on account with the winner's dentist of choice. All prize details will be determined by Sponsor in its discretion. Prizes may vary, in color, size and other respects, from items shown on Web site.. All expenses and costs associated with the acceptance or use of the prize are the responsibility of the winner.
Sponsor may, at its sole discretion and for any reason, substitute a prize of equal or greater value, including a cash award.
ELIGIBILITY: The contest is open to legal U.S. residents who are 18 years of age or older at the time of entry. Entries from non-residents of the United States of America are ineligible.
Employees of the Sponsor and the Sponsor's affiliates, the immediate families of such employees (employees' parents, siblings, children and spouses) and members of the same household of such employees are not eligible to enter the Sweepstakes.
All federal, state and local laws and regulations apply. No substitution or transfer of a prize shall be permitted except as provided herein. Specifically, no substitution, transfer, assignment or cash equivalent of a prize shall be permitted except at Sponsor's sole and absolute discretion.
Void where prohibited by law.
SWEEPSTAKES PERIOD/DEADLINE: The Sweepstakes commences on May 4, 2010, and ends on June 30, 2010 (the "Sweepstakes Period"). All entries must be received by the Sponsor by 11:59 p.m. Eastern Time on the last day of the Sweepstakes Period.

Entrants may only register once -- multiple registrations by the same Entrant may, at the Sponsor's discretion, result in ineligibility to receive the Sweepstakes' prize.
No correspondence or contact other than that specified in these Official Rules will be accepted as an entry in the Sweepstakes. All entry information becomes the property of the Sponsor, however, entry information will not be shared or sold.
HOW THE SWEEPSTAKES WORKS/PRIZE AND PRIZE WINNER SELECTION: One winner will be randomly selected from all valid entries that completely satisfy the entry conditions for the Sweepstakes during the Sweepstakes Period. No purchase is necessary to enter the Sweepstakes or to be awarded a prize. The Sponsor shall announce the winner of the Sweepstakes Prize by e-mailing the winning Entrant at the e-mail address entered and confirmed during registration, and/or by telephone via the telephone number provided on the entry form. The sponsor will pre-pay the dentist chosen by the winner $1,000 which may then be used by the winner as needed. The winner may divide the prize among no more than two dentists. The Sponsor reserves the right to announce the identity of the Sweepstakes prize winner and the prize awarded to the winner on any of its Web sites or those of its affiliates, including Facebook and Twitter, and via a press release to media.
The winner, who complies with these Official Rules, shall receive the prize within approximately eight (8) weeks after the end of the Sweepstakes Period. Each winner will be required to sign a Release Form confirming eligibility, release of liability and promotional participation. Except where prohibited by law, acceptance of a prize shall constitute and signify the winner's consent that the Sponsor may use the winner's name, likeness, image, voice, photographs, biographical information, address (city and state only) and written statements made by the winner about the Sweepstakes and Sponsor for promotional purposes, in all forms of media, in perpetuity and without monetary payment or additional consideration.
If a prize notification e-mail is returned or deemed undeliverable, or if the phone number is out of service, or if the winner is found to be ineligible or fails to comply with these Official Rules, or does not respond within 5 business days to notification of winning the prize, the Sponsor may decide, in its discretion, that the winner has forfeited the prize and an alternate winner of the prize will be selected from eligible Entrants.
ODDS OF WINNING: The odds of winning the prize depends on the number of valid entries received.
TRANSFERABILITY/TAXES: Winner is solely responsible for all applicable Federal, State and local taxes on the prize. An IRS form 1099 will be issued if required by law. No substitution, cash redemption or transfer of right to receive prize is permitted, except in the discretion of Sponsor. To receive prize, winner must execute an affidavit of eligibility and release of liability within 10 days of notification, if required by Sponsor and by law. Noncompliance or return of any form as undeliverable will result in disqualification and selection of an alternate winner.
CONDITIONS: The Sponsor is not responsible for errors, omissions, interruptions, deletions, defects, delays in operations or transmissions, theft or destruction or unauthorized access to or alterations of entry materials, or for technical, network, telephone, computer, hardware or software malfunctions of any kind, or inaccurate transmissions of, or failure of the Sponsor to receive entry information on account of human or technical problems in any combination thereof. The Sponsor reserves the right, in its sole discretion, to cancel the Sweepstakes in its entirety if it becomes technically corrupted or because of non-authorized human intervention. The Sweepstakes is subject to these Official Rules. By entering, (a) participants agree to these Official Rules, the decisions of the Sponsor, which shall be final and binding in all respects, and waive any right to claim ambiguity in the Sweepstakes or these Official Rules; (b) participants release the Sponsor, its directors, officers, employees, affiliates, agents, representatives, vendors and such persons' directors, officers, employees and agents from any and all liability for any claims, injuries, losses, costs or damages of any kind caused by participation in the Sweepstakes or resulting from acceptance, possession, use or misuse of the prize; and (c) PARTICIPANTS Acknowledge that the Sponsor and other released entities and individuals have neither made, nor are in any manner responsible or liable for any warranty, representation, or guarantee, expressed or implied, in fact or in law, relative to the prize, including but not limited to ITS quality, mechanical condition, or fitness, except for any standard limited warranty offered by any manufacturer of the non-cash prize. The Sponsor may prohibit an Entrant from participating in the Sweepstakes or winning the prize if, in its sole discretion, it determines that the Entrant is attempting to undermine the legitimate operation of the Sweepstakes by cheating, hacking, deception, or other unfair practices or intending to annoy, abuse, threaten or harass any other participants or the Sponsor's agents or representatives.
WARNING: ANY ATTEMPT BY A PARTICIPANT TO DELIBERATELY DAMAGE THE SPONSOR OR SPONSOR'S WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES (INCLUDING ATTORNEYS' FEES) FROM ANY SUCH PARTICIPANT TO THE FULLEST EXTENT OF THE LAW, INCLUDING CRIMINAL PROSECUTION.
USE OF REGISTRATION INFORMATION: By completing the Sweepstakes Registration forms, Entrants grant the Sponsor the right to use the information provided on the forms, and to contact the Entrant by e-mail, telephone, mail, certified mail or fax. All entrants will be subscribed to Quantum Health's free newsletter, It's Your Health. Entrants may unsubscribe at any time.
